We will be producing a booklet of services offered by participating service organizations to distribute at the Forum. To be included, you will be asked to provide the following information by September 10, 2009 with your Registration Form. You may want to start gathering the following information about your service organization in advance:

Name of Organization
Brief Mission Statement (75 words or less)
Contact Information (name, address, phone, email, website).
